Nevada agency for disabled, elderly receives $450K federal grant to support older adults, people with disabilities
CARSON CITY — The Aging and Disability Services Division recently received a federal grant of $450,000 from the U.S. Department of Health and Human Services to support Nevada Care Connection Resource Centers as part of the response to the COVID-19 pandemic.
Nevada Care Connection helps facilitate access to long-term services and supports for older adults, individuals with disabilities, caregivers, veterans, and families regardless of income. These services help Nevadans make informed decisions and access public and private services and programs.
To help Nevadans who have multiple needs during the COVID-19 pandemic and beyond, Nevada Care Connection has increased efforts to provide care coordination within the vast network of community partners, community members and agencies with the same mission statewide.
Resource and service navigators offer person-centered counseling to help individuals identify needs, explore their options, and develop a path to meet their long-term goals. Counselors help individuals navigate systems by providing services such as application assistance, pre-determining eligibility for public and private programs, and connecting individuals to services and resources.
